Rocky Mountain Gastroenterology accepts most major health insurance plans. Please remember that many health plans require a referral from a primary care physician. We recommend that you check with your individual insurance plan before scheduling an appointment to verify your benefit coverage.

PROCEDURE BILLING: Please download this form and use as a guide to contact your insurance company to understand your benefits, coverage, and procedure categories. You may receive invoices from separate entities associated with any scheduled procedure such as: RMG Physician; Pathology/Laboratory; Facility; Anesthesia. RMG will only provide information associated with RMG Physician fees. 

FACILITY INFORMATION: ALL procedure centers are managed by entities outside of Rocky Mountain Gastro. Therefore, RMG is NOT responsible for collecting facility fees for your procedure, as the fees are billed directly by the facility itself. If the facility has not contacted you prior to your procedure, we recommend you contact the facility directly for billing/cost information.

ANESTHESIA BILLING: Anesthesia services are billed separately by an anesthesia company. If your procedure is scheduled at Arapahoe Endoscopy Center, Aurora Endoscopy Center, Lakewood Endoscopy Center, North Denver Endoscopy Center, Crown Point Surgery Center, Centennial Hospital – please call 1-800-242-5080 for anesthesia and billing information.
